We are excited to announce the launch of the new Survey123 request for
assistance form!
As a benefit of the non-profit ArcGIS Online license, we have
been able to transition our request form from Wufoo to Survey123.
This also has the added benefit of being able to automatically map our
requests by city!
You will note that the updated Request for Assistance page has been
rebranded as Member Support.

Note: that only MAGIP members in good standing will be able to view the Survey123 form once they have logged into their account.
